A nice touching reflection on a very tough situation for Janeway. What nailed it for me was the sentiment of Janeway as being the loneliest person in the universe. This is very much a feeling that had struck me throughout the series and to an extent explains why she wouldn't chase a relationship with Chakotay over the ensuing years. She purposely cut herself off in many ways, in order [so she believed] to be a better captain, more able to get them home. Certainly, in the beginning she was more resolute in trying to cut herself off, but people like Kim, Kes and Seven drew her more and more out of that cocoon. Very good stuff.
